I just spoke to someone at Glaxo Smith Kline and they don't know why it's not on their site anymore. They said you can substitute a 3 by 5 card with your name, address, etc. in place of the form and your receipt and upc codes. For the $8 rebate remember you need 4 packages of Beeno. I'm a little worried about not sending the form but the woman said it would be accepted. This applies to everyone. She did not take my info and they are not making an exception for me or anything like that.

Is anybody getting emailed the pdf? I gave up on that since I heard the actual pdf has the person's name printed on it. However, if you go to the OP's link and to the Glaxosmithkline website, and email them requesting the rebate form, they will snailmail it to you.
Here is the direct email to get the form: consumer.communications@gsk.com if you need it.
As noted, some people were told they could use a 3 by 5 notecard instead, but as someone who as been turned down for rebates for stupid reasons, I would rather have the form.
